This assessment:

• Is not a medical diagnosis
  
• Is not a substitute for primary medical care
  
• Does not replace imaging, prescription management, or emergency evaluation
  
• Is intended to inform personalized lifestyle, nutritional, and supportive care recommendations
  

All recommendations are based on clinical interpretation and patient context.

2. Medical Care DisclaimerRHC does not diagnose or treat disease through this assessment.
You understand that:

• Any concerning findings may require referral to your medical provider
  
• You are responsible for following up with your primary care provider or specialist when medically indicated
  
• RHC does not order or interpret imaging (e.g., ultrasound, CT, MRI)
  

3. EligibilityBy purchasing this assessment, you confirm that:

5. Shipping & Sample Responsibility

• Test kits are shipped directly to you or provided through RHC
  
• You are responsible for:
  
  • Following collection instructions
    
  • Mailing samples promptly according to kit guidelines
    
  • Ensuring samples are not damaged or delayed due to improper handling
    

RHC is not responsible for invalid results due to collection errors, delayed shipping, or non-compliance with instructions.

6. Refund & Cancellation PolicyBecause this assessment involves laboratory processing and third-party testing, the following applies:

• Once a test kit has been shipped or registered, all sales are final
  
• No refunds are available once laboratory processing has begun
  
• If a kit has not yet been shipped or registered, refund requests must be submitted in writing and are subject to administrative fees
  

7. Results & Interpretation

• Results are reviewed by your provider within a reasonable timeframe after lab processing
  
• Interpretation is provided as:
  
  • A consultation (virtual or in-person), or
    
  • Written clinical recommendations, depending on your care plan

8. Genetic Testing Acknowledgment (If Selected)If you choose optional genetic SNP add-ons, you acknowledge that:

• Genetic testing provides risk and tendency information, not predictions or diagnoses

9. Privacy & Data Handling

• All personal health information is handled in accordance with HIPAA guidelines
